Release of Liability, Assumption of Risk & Hold Harmless Agreement
1. Assumption of risk. I understand that soccer is a contact sport and that participation in BAM United F.C. ("the Club") practices, games, tournaments, camps, travel and related activities involves inherent risks, including but not limited to sprains, fractures, concussions, heat-related illness, serious injury, and in rare cases death. I knowingly and voluntarily assume all such risks on behalf of my child.
2. Release and waiver. In consideration of my child being allowed to participate, I, for myself, my child, and our heirs and assigns, release, waive and discharge the Club, its owners, directors, officers, coaches, staff, volunteers, sponsors, and facility providers from any and all claims, liabilities, demands or causes of action arising from my child's participation, including those caused by ordinary negligence, to the fullest extent permitted by law.
3. Hold harmless and indemnity. I agree to indemnify, defend and hold harmless the Club and its coaches, staff and volunteers from any loss, liability, damage or cost, including attorney's fees, that may arise from my child's participation or conduct.
4. Medical authorization. I confirm my child is physically fit to participate. In an emergency, if I cannot be reached, I authorize Club staff to obtain first aid and emergency medical treatment for my child, and I accept responsibility for any resulting costs.
5. Code of conduct. My child and I agree to follow Club rules, show respect to coaches, referees, opponents and teammates, and understand that misconduct may result in suspension or removal without refund.
6. Media release. I grant the Club permission to use photos and video of my child taken during Club activities for website, social media and promotional purposes, without compensation.
7. Governing terms. This agreement is binding to the fullest extent permitted by law. If any part is held invalid, the rest remains in effect. I have read this agreement, understand it, and sign it voluntarily as the parent or legal guardian of the player.
